GUIDED DIVING

Each diver who is booking guided cavern or guided caves dives will need to fill out both a medical statement and a liability release. For divers who are able to answer “no” to each of the questions on the medical questionnaire, the basic form is sufficient. For divers who have a positive answer to any of the questions on the medical statement, the full form and a doctor’s signature is required. The liability release must be completed in full, and divers should fill out either “guided cavern experience” or “guided cave diving” in the “course or specialty field. All divers must still present a proof of certification upon arrival at the dive center.

COURSES
